While at the SNF (also known as a “nursing home”), you will be guided through your loved one's stay by Physical Therapists, Occupational Therapists, Speech Therapists and a Social Worker. You, as a family, will have a good idea of what type of setting will be the safest for your loved one within the first 2-3 weeks by attending interdisciplinary and family meetings. The potential discharge environments include: Long Term Care, Assisted Living, Board and Care, or Home.
