When a family member is hospitalized, you (the family) will be guided through the process by the hospital staff including the Primary Care Physician, Nurses, Therapists, and Discharge Planners.

If you want to become more informed regarding your family members diagnosis, the following web site contains health information, videos of surgical procedures and interactive tutorials to help ease any concerns: www.medlineplus.gov After the hospital stay, your loved one will either go Home or go to a Skilled Nursing Facility (SNF) (also known as a “nursing home”). The hospital’s Discharge Planner will provide you with a list of available SNFs. It is recommended that you tour the different facilities prior to discharge.
